Reporting Procedures

In order to report an Academic Integrity violation, faculty must do the following:

  1. Write a letter to the student.  The letter must include the problem, the sanction, and notification that the student has the right to appeal.

  2. Fill out the Academic Integrity Violation Report Form and the Academic Integrity Violation Sanction Sheet.  These forms can be found on the Academic Integrity website www.academicintegrity.eku.edu under “Faculty Forms”.  

  3. Give the student a copy of the letter and have the student to sign the bottom of the Academic Integrity Violation Sanction Form.

  4. Within 5 days of meeting with the student, mail the Academic Integrity Violation Report Form, the Academic Integrity Sanction Sheet, the letter to the student, and any other supporting documentation (e.g., a copy of the student’s paper, a copy of the website paper), to the Office of the Assistant Director of Academic Integrity, Turley House 1. All paperwork must be turned in.

If the student accepts the sanction, the case is closed but if the student desires to appeal the sanction, the Assistant Director of AI will meet with the student to discuss the violation and the appeals process. (The Assistant Director of AI may contact the faculty member for clarification and/or more information before meeting with the student.).

After the meeting, the Assistant Director of AI will send a notice to the faculty member confirming whether the student still desires to appeal the sanction.  If the student accepts the sanction at this point, the case is closed.  If the student still wants to appeal the sanction, the Assistant Director of AI will schedule a hearing with the student and the faculty member and the appropriate Academic Integrity Committee(s).
